PDF Files

The documents on this website are Portable Document Format (PDF) files. To view these documents, you will need Adobe Acrobat or the free Adobe Reader.

PDF forms may be used in the following manner:

  • Click on the selected form to open it using Adobe Acrobat or Reader.
  • Print the document on your local printer.
  • You can the "Save As..." command to save the document to your computer's hard drive.
  • Complete the form using a pen or typewriter.
  • Sign it if a signature is called for on the form, and forward the document to the proper jurisdiction via fax, U.S. mail or other method as indicated by your specific agency (some submittals require original signatures).

Microsoft Word Files

Microsoft Word files may be used in the following manner:

  • Click on the selected form to open it using your browser window.
  • Use the "Save As..." command to save the document to your computer's hard drive; or
  • Right click on the Word icon and select "Save Target As..." to save the document to your hard drive.
  • Start Microsoft Word and open the document.
  • Note that some versions of MS Word automatically reactivate the "Track Changes" feature for certain documents. If this happens, select "Unprotect Document" (files are not password protected), change the document view to "Final," disable the "Track Changes" feature, then restore the document protection while selecting the option to fill in forms data. (Note: Be sure to do this before you fill in any forms data, since some versions of Word will delete all entered forms data when document protection is turned on.)
  • Complete the form by typing in the fields onscreen (most forms are now configured for onscreen completion).
  • Save the document to preserve the data you entered.
  • Print the completed form on your local printer.
  • Examine the form for accuracy, completeness, and proper formatting. Go back to your computer to make any corrections.

When you are satisfied with your final copy, print it again, sign it if a signature is called for on the form, and forward the document to the proper jurisdiction via fax, U.S. mail or other method as indicated by your specific agency (some submittals require original signatures).
